A varicocele is the enlargement of veins within the scrotum, similar to varicose veins that occur in the legs. It can affect the blood flow to the testicles, leading to lower sperm production, decreased testosterone levels, and potential infertility. Symptoms of varicocele may include a noticeable swelling in the scrotum, discomfort or pain in the testicle, and in some cases, decreased testicular size. While some men with varicoceles experience no symptoms, others may experience fertility issues or a decrease in testosterone production, leading to sexual health concerns. Treatment typically includes surgery to improve blood flow and sperm quality.

Causes of Varicocele

A variety of factors can contribute to varicoceles. There may be a valve malfunction in the veins, preventing proper blood flow and causing veins to dilate and become enlarged. Variations in venous structure lead to poor blood flow or increased pressure within the veins, and swollen lymph nodes could block blood flow behind the abdomen.

Diagnosis

Healthcare providers may diagnose varicocele through:

  • Physical Examination: Checking for visible swelling or a “bag of worms” feel, which is characteristic of varicocele. The Valsalva maneuver is a breathing exercise that allows the doctor to locate enlarged veins while the patient is standing.
  • Scrotal Ultrasound: Confirming the presence of varicocele and assessing its severity by evaluating blood flow in the veins.

Treatment Options

Not all varicoceles need to be treated. However, some patients experiencing pain or infertility may benefit from:

  • Surgical Repair: A varicocelectomy removes or ties off the enlarged veins to improve blood flow to the testicles.
  • Embolization: A non-surgical procedure that blocks the affected veins using a catheter, redirecting blood flow to healthy veins.
